2026 Swine Lost 840 Tag Form

You must get your new 840 Premise ID Tag from your Breeder. Benton Franklin Fair will NOT provide you will a tag.

This form is to be filled out after you retain your new tag, so we can properly report the number to the Department of Agriculture.

New 840 Premise Tag number will be check before you go over the scale Aug. 24 Benton Franklin Fair weigh-in day!
